The mission of our Family Support Service, which utilizes case management services, is to reduce the stress on families by breaking down the barriers in access to care and services in their community. The intermediate goal is to provide support services to the family my locating resources in their community that will meet their particular needs. The Program provides help with planning for needs and finding resources; help with identifying and applying for appropriate government and community programs; funding for goods, services or home modifications that will help families stay together; and networking and support services.
